Dental Plan: Basic Overview
Diagnostic & Preventative care within plan terrms is covered at 100% (this includes things like oral exams and cleanings)
*Resorative care, oral sugery, periodontics and endodontics within plan terrms are covered at 80% (this includes things like fillings and root canals)
*Major restorative care and prosthodontics are covered at 65% (this includes things like crowns)
*Orthodontics are covered at 50%, with separate $1000 lifetime maximum.
There is also a TMJ benefit: treatment is covered at 65% subject to a $500 lifetime maximum.
The maximum plan year benefit is $1750.
*these items are subject to a once a year per person deductible of $75

Who can be covered?
Eligible graduate employees, their dependent spouses, domestic partners both same and opposite sex, and children up to age 26. No other family members are covered under the plan.
Find a Dental Provider
To use Delta's "Find a Dentist" link, follow these steps: 1) choose between a directory of dentists in MA or a nationwide directory (yes, we have nationwide coverage), 2) enter our plan type, Delta Dental PPO Plus Premier, 3) then enter your zip code and the type of dentist you wish to see, 4) the site should then generate a list of provider options for you.

Costs?
The dental plan is free for eligible individuals. It is $100 to add your family members. This can be paid for with a check, via Paypal or by University payroll deduction (fall only).
