There are only two situations that might require you to make payments to the Trust Fund: 1) if you opt for the family dental plan, you will need to pay the $100/year premium or 2) if you become ineligible for the plans and opt to extend your coverage via the COBRA plan.
Family Dental Plan Premium Payments
You can pay the $100/year premium using one of the following options: 1) University Payroll Deduction (only an option through 10/5/11), 2) e-check or credit card via PayPal.
COBRA Payments
COBRA payments are due on the 1st of each month and can be paid either monthly or several months in advance, at your discretion. You can pay using either: 1) PayPal e-check or 2) PayPal credit card payment.
Payment Instructions for all methods
University payroll deduction:
1) Simply fill out and return the University payroll deduction form to GEO (201 Student Union) by October 5th. The $100 payment will be deducted in $20 increments over five pay periods..

1) Please note that if the name on the PayPal account is someone other than the eligible graduate employee, the eligible grad employee's name and student id should be indiated during the Paypal process (there is a field for this).
