SUMMARY OF MATERIAL MODIFICATION TO
THE SUMMARY PLAN DESCRIPTION

This summary of material modification (SMM) describes changes to the UMass Health & Welfare Trust Fund's Delta Dental Plan (Plan) and supplements the Summary Plan Description (SPD) for the Plan. The effective date of each of these changes is indicated below. You should read this SMM very carefully and retain this document with your copy of the SPD for future reference.

Summary of Benefit Changes:
1. There are no benefit plan changes for plan year 2011-12.
Summary of Administrative Changes:
1) Open Enrollment Periods: during plan year 2011-12, there will be four discrete open enrollment periods with deadlines associated with each. Online enrollment must be completed and submitted by thelast date of an enrollment period if order for that enrollment to be processed. If an enrollee misses an open enrollment period deadline, it will not be possible to complete their enrollment until the next open enrollment period.
These open enrollment periods for 2011-12 are (last date in the period is the deadline):
* September 21-October 5 for guaranteed 11/1/11 coverage
* November 30-December 14
* February 1-February 15
*April 30-May 14
No exception can be made to these deadlines.
2) Payment options for the family dental plan and COBRA plans: As of plan year 2011-12, the Trust Fund will no longer accept hard copy checks. Payment options for the family dental plan will include: 1) University payroll deduction prior to 10/5/11 and 2) e-checks or credit card payments processed via Paypal. Payment options for the COBRA plan will include: e-checks or credit card payments processed via Paypal.
3) Application process: during plan year 2011-12, there will no longer be a requirement that enrollees sign a hard copy authorization form for dental & vision benefits. Instead, enrollees will be asked to provide an electronic signature on a pdf, which is sent to the benefits administrator and an email confirming the application will automatically be sent to the enrollee.
