Basic Overview
For plan year 2011-12, you can submit receipts between Nov 1, 2011 and June 30, 2012.
Eligible grad employees can be reimbursed for up to $80 per year towards your expenses for wellness/fitness programs. There is no reimbursement for receipts for family member's wellness/fitness costs.
The wellness reimbursement is accessed through the same online enrollment process as the dental & vision plans, except that you must include a copy of a valid receipt demonstrating your payment of membership fees/fees/tuition to an institution or organization.
This benefit follows the same plan year of Nov 1-Oct 31 each year. If you are eligible for at least one 6-month period of dental & vision, you are eligible for a maximum of $80 per year in reimbursements.
Eligible Wellness Activities
Physical activity programming that promotes exercise, fitness and stress reduction through exercise and fitness. Examples of eligible programs include: a gym membership, a yoga class, a dance class, a martial arts class, an aerobics class. Both on campus and off campus programs are eligible. Yearly membership fees, monthly service fees and equipment fees associated with a gym or classes are eligible; late fees are not. New this year!: receipts from nationally recognized weight loss programs with an exercise component will also be eligible for reimbursement.
When to turn in your receipts
Reimbursements are made for purchases made during our plan year, 11/1/11 through 10/31/2012. If you pay on a monthly basis, only charges dated after 11/1/11 are eligible. If you purchase semester or yearly plans, the period of the receipt must include at least part of the plan year to be eligible (i.e. UMass gym memberships for fall semester are eligible).
How to Submit Your Receipt
Even if you are declining dental & vision benefits, you still need to register on our website at hwtrust.geouaw.org for the wellness reimbursement. Once you complete the online form, a personalized wellness form will be created for you in pdf. Print this form and attach your receipt and return these to GEO, 201 Student Union.
How & When You'll Receive Your Reimbursement
There are 2 ways to receive your reimbursement:
1) the fastest option: provide either your email or your cell number and indicate you'd like to be paid via Paypal, and you will receive an email/text when your reimbursement is available at Paypal. Once at Paypal, you can login and transfer your payment for free to any bank of your choosing or if you prefer, request that Paypal send you a check for a fee of $1.50.
2) the slower option: opt to receive a check directly from the trust and you can pick up your check on predesignated dates that will be sent to you via email.
Maximize Your Reimbursement
You can get a discount on some gym memberships by registering for AETNA navigator (your student health insurance provider) and using their Global Fit network.
